Account Deletion Request Form

Use this form to request deletion of your Pavilion account and associated personal data for a Pavilion-powered club app or service. If your account is connected to a club, Pavilion may need to coordinate with that club to verify and process the request. Some records may be retained where required or permitted for legal, security, accounting, dispute-resolution, backup, or legitimate club operational purposes.
